The card pokes fun at dissociative identity disorder. "Double Heather" is a pun of "double header" which most commonly refers to baseball games played back to back, or could simply mean "having two heads"
Trivia[]
In some prints of the original second series, the card's b name was changed to Fran FRAN, possibly because Topps felt that using the word schizophrenia would be too controversial. As a result, card 49 is the first Garbage Pail Kids card to have more than two names.
